Scheduling Appointments and Registration
The easiest way to schedule an appointment is to contact our reception staff at the following locations:

For those who wish to contact Bone & Joint Surgery Associates, S.C. via the Internet, please click on appointment request form, complete the requested information and a receptionist will contact you by telephone to schedule an appointment. When you arrive for your appointment, you will be asked to complete a patient registration form. You can print a copy of this form by clicking on registration form. Completing this information in advance and bringing it with you at your scheduled appointment will minimize delays and ensure that all required information is fully completed.
Although every effort is made to see you at your scheduled appointment time, emergencies can occur and delays may be inevitable. A sincere effort is made to adhere to the appointment schedule as closely as possible. In the event you are unable to keep your scheduled appointment, please call at least 24 hours in advance to cancel or reschedule your appointment.

Office Hours
Bone & Joint Surgery Associates, S.C. office hours in Madison are Monday through Friday 8:00 AM - 5:00 PM. Clinic hours in Fort Atkinson and Columbus are also Monday through Friday 8:00 AM - 5:00 PM for scheduled appointments, but hours may be subject to change on days without a scheduled clinic. If you are unable to reach reception staff at these offices, please contact our Madison clinic at 608-238-9311 to assist you with any issues or concerns. In addition, 24-hour emergency medical care is available by contacting one of our physicians through our answering service at 608-238-9311.

Financial Arrangements
As a courtesy to our patients, Bone & Joint Surgery Associates, S.C. will prepare and submit insurance claims in your behalf provided the necessary billing information has been supplied. Please be advised however, that this does not alter a patient's financial obligation for all charges incurred. If you do not have insurance coverage, a pre-payment will be required at the time of the appointment. Please contact our Patient Financial Services department at 608-238-9311 to make payment arrangements prior to your scheduled appointment. Clinic policy requires at least partial payment on all office visits and 75% pre-payment for any scheduled surgery. Please be advised of the following:
Bone & Joint Surgery Associates, S.C. accepts assignment on all Medicare claims.
Patients eligible for Medical Assistance must present valid documentation of coverage at each scheduled appointment.
Patients receiving care for a work related injury should be prepared to provide the name and address of your employer's workers compensation carrier at the time of service. Personal health information must also be provided in the event that the claim is denied.
